Athletics Physical Form and Procedures

Students must meet these minimum requirements to play sports and complete the following:

  1. Complete the online Check-Off Sheet
  2. Students must have and maintain a minimum 2.0 GPA
  3. Download and review the Student Athlete Accident Insurance Program brochure
  4. Purchase Athletic (Interscholastic) Insurance
    • Athletic Insurance is $18
    • Football (Fall Football Plan Insurance is $63.00)
  5. Complete, sign and notarize the Athletic Physical Packet (Abnormal EKG Clearance Form, ECG/EKG Clearance Form) 
  6. Complete IMPACT test (see Athletic Trainer, Mr. Gonzalez)
  7. Students must completely watch the following videos from NFHS Learn and print certificate of completion:
    • Concussion in Sports
    • Sudden Cardiac Arrest
    • Heat Illness Prevention
    • Sportsmanship

Steps to purchase Athletic Insurance ($18.00):

  1. Login to Parent or Student Portal (www.dadeschools.net/login)
  2. Click on Apps/Services/Sites
  3. Click on OSP (Online School Payments)
  4. Click on Secondary Schools
  5. Toggle until you find North Miami Beach Senior High School 7541
  6. Click on box to select the correct insurance coverage
    • Athletic (Interscholastic) Insurance $18
    • Football Fall Insurance $63
  7. Click on Add to Cart
  8. Click Check-out
  9. ​Select Student Profile
  10. ​Click Next
  11. Complete billing information
  12. Agree to payment terms
  13. Verify information is correct and that correct insurance plan is selected.
  14. Complete purchase
  15. Print receipt
Eligibility for Participation.
Non-Traditional students, as defined in Bylaw 1.4.31, are students who are not enrolled in and do not physically attend the school at which they participate. These include home education students, Personalized Education Program (PEP) students, charter school students, approved alternative/special school students, private school students, virtual school students, and traditional public school students. You must PRE-REGISTER intent to participate. (Click here).
In accordance with Florida Senate Bill 538 (click here to view Senate Bill Summary) Miami-Dade County School Board Policy 2431 (click here to view - Item VIII letter B) require applicable private school and Personalized Education Program (PEP) students participating in an M-DCPS interscholastic athletic sport or qualifying activity to pay a $400 fee for each sport or activity. This fee is mandatory and applies uniformly throughout Miami-Dade County Public Schools (M-DCPS).
The $400 fee may not be waived or reduced.
No private school or PEP student is exempt.
Students who have already begun participating in a fall sport or qualifying activity must also be assessed the fee.
The fee must be collected for each sport or qualifying activity in which the student participates.
Payment and Deposit Procedures:
Due to Non-traditional students not having access to the OSP electronic payment platform, payments must be collected at the school site by cash or cashier's check.

Once all of the above required documents which pertain to the student-athletes particular case are received in the athletic office, at that point GPA (grade point average) will be checked and verified. If the GPA is above the required minimum of 2.00, then and only then can the athletic insurance premiums be collected.
Insurance for all sports except football is $18.72. Fall football insurance is $65.52. Insurance is mandatory as per district policy. Insurance can only be purchased in the athletic office on campus. Instructions for payment of required insurance will be provided at the time all necessary documents are submitted in the athletic office.
Incoming freshman, new students-athletes to Coral Gables Sr High and students new to Miami-Dade County Public Schools have a separate set of requirements for insurance coverage for summer workouts and practices. Please visit the section titled "I want to play, what do I need"
