Athletics Physical Form and Procedures

Students must meet these minimum requirements to play sports and complete the following:

  1. Complete the online Check-Off Sheet
  2. Students must have and maintain a minimum 2.0 GPA
  3. Download and review the Student Athlete Accident Insurance Program brochure
  4. Purchase Athletic (Interscholastic) Insurance
    • Athletic Insurance is $18
    • Football (Fall Football Plan Insurance is $63.00)
  5. Complete, sign and notarize the Athletic Physical Packet (Abnormal EKG Clearance Form, ECG/EKG Clearance Form) 
  6. Complete IMPACT test (see Athletic Trainer, Mr. Gonzalez)
  7. Students must completely watch the following videos from NFHS Learn and print certificate of completion:
    • Concussion in Sports
    • Sudden Cardiac Arrest
    • Heat Illness Prevention
    • Sportsmanship

Steps to purchase Athletic Insurance ($18.00):

  1. Login to Parent or Student Portal (www.dadeschools.net/login)
  2. Click on Apps/Services/Sites
  3. Click on OSP (Online School Payments)
  4. Click on Secondary Schools
  5. Toggle until you find North Miami Beach Senior High School 7541
  6. Click on box to select the correct insurance coverage
    • Athletic (Interscholastic) Insurance $18
    • Football Fall Insurance $63
  7. Click on Add to Cart
  8. Click Check-out
  9. ​Select Student Profile
  10. ​Click Next
  11. Complete billing information
  12. Agree to payment terms
  13. Verify information is correct and that correct insurance plan is selected.
  14. Complete purchase
  15. Print receipt
